Autumn Care of Forest City is nestled in the scenic foothills of the Blue Ridge Mountains in Rutherford county. The facility is conveniently located within a short drive from 4 major hospitals known for their outstanding heart, cancer, and trauma centers. Additionally, Rutherford Hospital, a provider of general health services is just minutes away.

Established in 1967 as a rest home, in 1990 the building was expanded with an addition of 60 skilled nursing home beds. In 1998 the facility added 40 skilled and rehabilitation beds. Autumn Care of Forest City currently offers 100 skilled and rehabilitation beds and 28 rest home/assisted living beds. In 2003, out-patient therapy services were offered to the community. We are proud to be known as the “Rehabilitation Specialist” in our area.
