Liberty Commons Nursing & Rehabilitation Center of Halifax County is located in scenic northeastern North Carolina near the Roanoke and Tar-Pamlico River basins. Offering compassionate nursing care and quality rehabilitation services, Liberty Commons has 50 skilled nursing beds and 25 assisted living beds.
With a reputation for providing outstanding rehabilitative services, Liberty Commons offers residents the ability to move from service to service as needs change. Assisted living residents enjoy priority access to long-term care and rehab, Long Term Care residents have access to rehab therapies to help achieve their maximum level of functioning.
For the past two years, Liberty Commons has enjoyed outstanding state surveys and was recently awarded the 2007 Quality Award by the AHCA/NCAL.
