Touched by Angels is a Home Healthcare
agency and was founded in 1998 by Barbara
Reece, RN. Barbara is an RN and has many
years of home healthcare experience.

Touched by Angels expresses genuine concern and compassion for
our patients, treating them as individuals, not as an illness or
disability.

Our "Special Touch" provides physical and emotional support for our
patients AND their families.

We offer personal care and domestic services on a temporary or a
permanent basis.

Our Registered Nurse is on call 24 hours a day, 7 days a week for
special patient needs.

We believe that it takes special people to provide care in the home
setting - people who create a warm environment, demonstrate
compassion and an eagerness to serve.

Touched by Angels extends our "Special Touch" to our employees,
creating an atmosphere which enables them to feel valued, respected
and an integral part of this company.

We provide highly skilled Certified Nursing Assistants, Sitters,
Companions, Chore Workers and Habilitation Technicians.

Our employees are screened, bonded and insured.

MR/DD Services Available:

CAP-MR/DD

Personal Care/Enhanced Personal Care - Services include support, supervision and
engaging participation in personal care tasks for individuals who, due to a
developmental disability, need help with eating, bathing, dressing, personal hygiene
and other activities of daily living. They also include assistance with monitoring health
status and physical condition, assistance with transferring, ambulation and the use of
special mobility devices. Enhanced Personal Care is for individuals who have intense
medical or behavioral needs.

Home and Community Supports - This service provides habilitation, training and
instruction to achieve outcomes identified in each individual's plan of care. Goals
targeted may involve daily living skills, community participation and other functional
skills needed to increase independence.

Respite Care/Enhanced Respite Care - Respite provides relief to caregivers
exhausted from the stress and time consumed in meeting the physical and emotional
needs of caring for a disabled loved one. This is a temporary, intermittent care service
for people with disabilities provided by a qualified caregiver. Enhanced Respite Care is
for individuals who have intense medical or behavioral needs.

Supported Employment - This service provides supervision, training and supports to
assist individuals with developmental disabilities to acquire and maintain competitive
employment at or above minimum wage within the community.

Caregiver Training & Education - Education and training about the nature and
impact of the developmental disability, effective intervention strategies, community
integration options, and the use of specialized equipment and supplies.

DEVELOPMENTAL THERAPY

This is a state funded service that includes individually designed instruction, training or
functional developmental intervention activities based on the assessment of and unique
strengths of the individual child or adult. Services are designed to support the
individual in the acquisition of skills that the recipient has not gained during the
developmental stages of life and is not likely to develop without additional training.

For children, services focus on strengthening skills in the major developmental
domains and may include training and activities in areas such as self-help, language
and cognitive development and psychological skills.

For adults, it may include training in activities to strengthen appropriate developmental
functioning in areas such as self-care, mobility, socialization, independent living and
self-advocacy and rights.

MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES

Community Support - This service consists of mental health and substance abuse
rehabilitation and supports necessary to assist the person in achieving and maintaining
rehabilitative, sobriety and recovery goals. This service includes Case Management to
arrange, link or integrate multiple services and assess the recipient's needs. It also
includes one-on-one services in the home and community to address financial, social,
educational, vocational and housing needs. Interventions are strength-based and
focused on promoting recovery, symptom reduction, increased coping skills and
achievement of the highest level of functioning in the community.

Diagnostic Assessment - Touched by Angels contracts with other agencies to
complete Diagnostic Assessments as needed. The Diagnostic Assessment is an
intensive clinical and functional face-to-face evaluation of an individual's mental
health, developmental disability, or substance abuse condition that results in the
issuance of a report with a recommendation regarding whether and individual meets
target population criteria and includes an order for Enhanced Benefit Services that
provides the basis for an initial Person Centered Plan.
